Care2x

Care2x is a free, open-source integrated hospital information system for hospitals and health centres that want admissions, wards, outpatient clinics, laboratory, pharmacy and surgery in one PHP web application. You host it on your own server and staff use it from a browser. Most searches for “care2x” are about the Care2x download, its GitHub code, the demo and the login screen, and all of that concerns the general hospital system.

There is no Care2x Dental Fork. Nobody has published a dental branch of Care2x, and the code has no dental charting, periodontal records or dental treatment planning. For a dental department inside a hospital, Care2x can hold registration, appointments, outpatient notes, lab requests and pharmacy orders; tooth-level records need a separate system. The project is also largely dormant: the last stable package is 2.6.27 from January 2012 and a 2.7 alpha followed in March 2015. A community GitHub repository has continued with PHP 8 compatibility work, with commits as recent as December 2025, but without a tagged release.

1. Send pre-operative blood tests through the laboratory module.

Install and first run

• Choose the code line: the 2012 stable package targets PHP versions of its time; the GitHub master branch is where recent PHP 8 compatibility work happened. For a new test install, start with GitHub.
• Server: a web server with PHP and MySQL, typically Apache on Linux.
• Steps from the README: extract the code into the web server directory, create the cache directory if it is missing, make the cache, installer and uploads directories writable if the installer needs it, then open the site in a browser and follow the installer.
• Demo: the GitHub README points to a public demo instance, which is the quickest way to see the interface before installing.
• Gotchas: there are no tagged releases on GitHub, so pin the exact commit you deploy; lock down the installer directory after setup; and do not put real patient data on an untested installation.

Limitations

• No dental fork or dental module: no odontogram, periodontal charting or dental treatment plans.
• Last stable release in 2012; the 2015 build is an alpha.
• Recent GitHub work is maintenance (PHP 8 compatibility), not new clinical features, and there are no versioned releases.
• No dental insurance claims processing for the US, UK or Canada.
• Documentation on SourceForge is old (manuals folder last changed in 2007).
• Needs a self-managed server, backups and security updates. No regulatory clearance is claimed.

Between the two free hospital systems, OpenClinic GA has more recent packaged releases and ready installers, while Care2x has more interface languages and recent PHP 8 work on GitHub. Neither replaces a dental practice system.
